Medical electronics change slowly and are documented heavily. A board that goes into a device has to be identical to the one that was validated, which makes revision control and a stable surface finish more important than the last five percent of cost.

Long storage
Boards are often built well before assembly. Immersion gold does not oxidise, so a board built now still solders in six months.
Sealed operator panels
Membrane keypads need a flex circuit with plated or carbon contact pads and coverlay sealing everywhere else.
Moving joints
Signals crossing a hinge or a sliding stage need a flexible circuit rather than a harness that will fatigue.
Revision discipline
Each build has to reference one released Gerber revision and one released BOM revision, recorded together.
