A raised toilet seat bolts or rests on the existing toilet. It is the right tool when the bathroom is already reachable.

A commode chair has its own bucket or a drop seat over the toilet. Use it when the user cannot walk to the bathroom at night, or when the toilet height and arm support cannot be fixed in place.
